A temporary instruction permit (TIP) is similar to a Louisiana State Identification Card as it is a form of legal identification but it also allows for an individual to participate in a Louisiana approved driver’s education school. Without the TIP card, a person will not be allowed to sit in on any classroom or behind-the-wheel driving instruction.
The temporary instruction permit is only available at the Office of Motor Vehicles throughout Louisiana and NOT through an Express or TAG office.

Public, Private or Charter School Program
A school enrollment form is a verification certificate that is required by the Office of Motor Vehicles for all minors that indicates he or she is currently attending or is enrolled in junior high or high school.
Every junior high and high school is different, but typically the enrollment form can be obtained from the school’s front office or from the guidance counselor.
As of now, any student attending Walker High School can request the enrollment form online by visiting: Walker High School’s Online Enrollment Form Request.
*If other high schools within the area open an online request for the enrollment form then we will add those schools to this section.
Homeschool or Home Study Program
If the child is homeschooled then he or she will need the registered nonpublic school confirmation letter as well as an enrollment form which can be found by visiting: Homeschool or Home Study Program Enrollment Form.
There are several ways to request a Louisiana birth certificate replacement. For the most up to date and accurate information, please visit: Louisiana State Registrar & Vital Records.
Requesting a replacement of your social security card has never been easier. To do so, please visit: Online Social Security Card Replacement.
